  • Commissions, Off duty, self-employment (1099-MISC)
  • Sales of stocks or bonds (1099-B)
  • Self-employed business income & expenses (1099 MISC, 1099-K)
  • Retirement income: IRA,  Pensions, Annuities (1099-R)
  • Stocks, Mutual Funds and other investments (Capital Gain and Losses /Schedule D)
  • Lottery or gambling winnings/losses (W-2G)
  • Alimony paid or received
  • Education expenses (1098-T) & student loan interest (1098-E)
  • Payments from qualified education programs (1099-O)
  • Rental property income and expenses (Schedule E)
  • Record of purchase or sale of residence (HUD and /or 1099-S)
  • Medical and dental expenses
  • Real estate and personal property taxes paid
  • Estimated taxes or foreign taxes paid
  • Cash and non-cash charitable contributions
  • Mortage or home equity loan interest paid (1098)
  • Prior year tax return if new client (not necessary)
  • Social Security cards (taxpayer and dependents)
  • If using bank products : Identification information -Confirmation of Social Security number (in person only) 2 ID's are required : copy of prior year Tax return,  goverment issued social security card, current military ID or 2018 IRS mailling label.
  • Taxpayer identification : Drivers License, Passport (US o Foreign), State ID/Tribal ID, Resident Alien ID, Military ID, Matricula Consular Card.
